Can it really be the case that Truss was not warned about the dangers of her actions, that the majority of them were wrong or that advice they gave was ignored? Surely all three possibilities are bizarre. Which leaves what?

I think, in fairness, the "fiscal event" is the responsibility of the person who issued it, Kwarteng. Prime ministers over the years have selected their chancellors, discussed policy with them, and stood by them at least as long as the next reshuffle, but they don't usually take direct responsibility for budgets.
The chancellor (and his team) never just thinks up a budget and delivers it without the PM being involved all along, and ultimately signing it off. The PM is as responsible as him.
Truss and Kwarteng are both contributors to Britannia Unchained…. a book which reads like an undergraduate essay written four hours before the deadline….
