I would do it thus:

First International Conference on Health Promotion

Ottawa, 21 November 1986 - WHO/HPR/HEP/95.1
website http://www.who.int/hp...ottawa_charter_hp.pdf accessed 15 May 2011

This is how I was taught to do it when I did my degree, it's important to show the full web address and the date you accessed it, as the detail there may change if anyone checks it out later.

sorry the layout went a bit skew-whiff, I'd do it like this:

I would however check with your uni - you should have had guidance on web referencing, as some may do it differently - ask your student support unit if in doubt.
