You could make a grand gesture - take her out for a winter picnic, or to a show, or something else she'll enjoy.
DO NOT buy her underwear (it won't fit, plus she'll either think it's too tarty or too dowdy), perfume (unless you're absolutely certain of her favourite) or anything 'domestic', eg. a DVD player, TV, set of saucepans etc etc. You also take a risk buying clothes generally, for the same reason as underwear).

I have asked for a massage session, my boyfriend got me a session at clarins one year it was fabulous. I disagree with the cuddly toys, i wouldnt be up for that and with regards for underwear - i love receiving it! A classy set from Knickerbox is always nice, and despite its reputation Ann Summers sells some lovely sets. Depends what you're girlfriend is like really. If she wears a lot of jewellery like beads and earings etc. M&S have some gorgeous jewellery boxes which have been quite hard to find over the past few years. Clothes are always good too!
