Earphones for small ears

Regular earphones like the ones which come with iPods are too big and I can't get them in my ears. I currently have a pair of the apple ear bud earphones with the adjustable outers but even the smallest one of these is too big for my ears and they fall out even when I am sitting still.
Can anyone recommend any good earphones which could fit. I have tried the ones also which hook over your ears but I'm not so keen on them.

They are so comfortable and they fit so well that you can turn the volume up really loud and no-one sitting next to you can hear it at all.
