When did heaven/hell start, and what's their population?

Was it when religion started to gain momentum a couple of thousand years ago or before that? And at what time did people start going to either place? Also are there any estimates for the population of both places combined?

The Bible states that Jesus will return with thousands and tens of thousands, not millions, so considering the millions who live and have lived, an awful lot of people are going to miss out on heaven.
