Help please. How do I insert a header/footer?

Do this - then choose 'view' from the menu bar - and select the 'form header/footer' option.
This should create header/footer areas in the design view of your form.
A tool bar appears on the desktop beside your form when you are in design view -
one of the buttons is a text box function (NOT the label function).
Click this button and use the cursor on the header/footer areas to create text boxes you can enter text into and format as you desire.
