Is there an idiot's guide to moving my music from one computer to another?

I currently manage my iTunes on my work PC but have now, for the first time ever, bought myself a computer for home, and I'd like to transfer my music library.

I've heard numerous horror stories of people trying this and their whole collection just disappearing into a black hole!

flip-flop - I honestly don't think you can do it but I may be wrong.

I would google it and there may be a few software products out there but I never heard of anyone doing it.
