http://www.winpatrol.com/
This runs all the time in the background and it monitors if anyone is secretly trying to install a program or change one of your settings like your browser home page.
A little window comes up saying someone is trying to install a program or change a setting and you have the ability to say yes or no.
Very useful as it can stop viruses and other rogue programs BEFORE they get on your computer.
I also run adaware which you have to run every so often but it checks for spyware. Get it here

Both programs are free.

most people use these programs in conjunction with each other. Not many programs get rid of or protect you from everything, so using more than one helps. I have Avast Antivirus and Ghostwall firewall for security and virus protection. Crap Cleaner, Ad-Aware SE, Spybot, Stinger and regcleaner to get rid of all the other crap. I run those and then if everything is running perfectly, I turn off system restore and then turn it back on again and set a new clean restore point.
