It depends on your bank. You need to ask them.
You do in the States $2.50 everytime you withdraw cash plus the exchange rate and any charges your bank adds to it!
Nearly always yes unless you are with a bank that has it as a perk. the Nationwide used to but not any more. Don't know who does now.

Ah read further. Debit is a "no no".
I've occasionally used a bank's cash machine in the US and not been charged by anyone but that may have changed as far as the US banks are concerned. My bank, Nationwide, doesn't charge depending on the card used.

Nationwide seems to be one of the better ones.
When looking at the tables in O_G's link, it's worth remembering that the exchange rate used for the transaction will be the standard inter-bank rate. If you go to an exchange facility (such as the Post Office here in the UK or a bureau de change abroad) you typically get a rate about 5% poorer than you do from a cash machine. So, despite the charges, you can still be no worse off (and possibly rather better off) by using ATMs abroad.
