the batsman could be Bradman in his final innings, which meant his average was 99.6 and thus failed to average 100.

Bradman's final innings was a two-ball duck, rather than a conservative four in a session.

As to question 1 is that a 3 or a 5 game series?

I can tell you the most maiden overs bowled in a test match is 75 by Alf Valentine for Jamaica against England at Lords in 1950.He bowled 28 in the first innings and 47 in the second. Not only that but his bowling partner Sonny Ramadin delivered 70 maiden overs in the same match.
