Does anyone know of a company that does car hire in the US but shows all their charges, especially insurance up front on the booking process so I can not finish up with all the add ons later.

They are upfront about costs and they are in the UK so you can talk to someone first. It's worth considering getting a separate policy to cover insurance excess, it can be cheaper depending on how long you're going for and how often you hire a car. We've never had trouble with US car hiring in 30 years, they seem less devious than their European counterparts.

It is important to understand that USA car insurance arrangements differ significantly from the European norm. In the USA it is normal for the hire driver to be covered by his own policy, not the car hire firm's, and therefore taking out ("additional") insurance with the company tends to be rather expensive and sometimes very difficult to arrange. zebo is quite correct about excess insurance and for years I have used the same people he mentions, except that their North America policy cover is far more extensive than in the European or even "Worldwide" version, exactly for the reason I mentioned - as you will discover when you check the details.
