In most cases poor hygiene. I've walked in, and straight out again of many pubs.
Regarding the hygiene in most pub toilets to put it mildly they always stink to high heaven.

Only last week I popped into what appeared to be a nice well kept bar for a coffee, until I saw a young lad come round wiping the tables with a dirty cloth. He worked his way round half a dozen empty tables without any cleaning liquid whatsoever.

It looks like antibacterial sprays have been put to the back of the cupboard never to be seen again.
They won't be seeing me again either.
