spayed cat in season

my cat was spayed 2 years ago but has now come into season again. the vet suggested she may have had 3 ovaries. has anyone heard of this before?

I believe that it is possible for a spay to be incomplete and sufficient hormone producing material to be left for this to happen, but I am not an expert
