sparkplugs you've probably already thought of this but are your little girl's undies pure cotton or do they have any lycra/elastane in them at all, even a tiny bit? Lycra makes me itchy in the delicate areas, although I can tolerate it in bras , t-shirts etc.
And do you wash them in gentle detergent?

My 5 yr old also had an itch, we swapped soap for plain water - didn't work. I use only Fairy non bio and she had creams and all sorts from our GP
It then became partly habit and partly itch. We were out one day last summer and she was talking to a little girl and scratched herself and it turned out that the other little girls mum had also had the same problem with her daughter. What cured both her daughter and mine was a change of underwear - not from nylon/lycra to cotton but the style of undies. My daughter now wears those shortie style pants (cheap from ASDA) and we have no itching
