How to clean a cotton cambric pillow?

I have a rather old pillow
cover 100% cotton cambric
Filling: polyester fibre
cleaning instructions are basically
don't wash don't dry clean don't tumbledry don't bleach
It says I can sponge clean with a damp cloth... thing is this thing has had a pillow protector on it for years (obviously washed the protector regularly..) but i'm sure dust and dirt and bedbugs and things can still get through that so how would you recommend giving it a really thorough clean and making it smell nice and fresh? It's annoying because all my other pillows can be put in the machine!
