A blow torch might be simplest. (NB: Wear a mask, because there's a risk of toxic fumes being released).

Have you tried using a high-pressure washer?
Paint stripper
What kind of paint?
Try one of them power washers you can hire them. Not the small electric plastic ones you get from B & Q or Argos, The high power 2000psi petrol ones that if you hold close enouth can actualy burn through the concreate it's self! Then just stand back and hold it so it just gets off the paint! Don't get a weak 100 or 200psi power one, You need at least 1000psi Like they use on the farms for cleaning the pig pens out!
Just a word of warning: When concrete is heated (anything but really quite slowly) to high temperatures, such as by a torch, it is likely to exhibit explosive spalling. Power washers can, however, not wash through concrete in the sort of timespan people are prepared to be there watching, unless it is unworthy of the name (i.e. it is about to crumble anyway), no matter how powerful the washer. Paint stripper application followed by wire brushing, repeated several times, followed by washing, should do the job.
............ just thought I'd throw another solution in, Dibbers ................
Power washer with a sandblasting attachment (you can rent them) :o)
