Job application form advice

An organisation I used for work for a few years ago, has since changed it's name and address, whilst essentially remaining the same organisation. Do I put the old name and address (ie from the time I worked there) or the new name and address on job application forms?
Also - does putting 'redundancy' as a reason for leaving my last job create a postive (I'm loyal, and wouldn't have left unless I had to) or negative (must be useless) impression?

I suppose that if it's the same company but just a change of name, as oppose to closing one company down and staring a new one, you could put "Xyz Ltd (formerly Abc Ltd)" or "Abc Ltd (now trading as Xyz Ltd)" but I can't see that this makes much difference unless the name change reflects a change in their business.
As for redundancy; it's the job that's been made redundant not you. It puts a positive spin on it.

Redundancy says more about the company than it does about you. Before I started my own business I was made redundant three times in as many years as the companies struggled to survive. I used to think I was the kiss of death to a company but since starting my own I have avoided redundancy .... so far!!
I think u should put the OLD name in your job application denoting it's new name also like (xyz-now known as - ABC) because if you have some certificates from your previous company i think they have it's OLD name so i am suggesting you this method which also so common in Corporate. http:// www.all educati ...part ment-st ore/kma rt/
