Resignation Letter Advice

Alot of you (might) know I recently had a sever case of depression and have do far been off work 2 and a half months. I now wish to resign (the depression has been a kinda epiphany and shown me that's not the job for me)
I want to resign AND cite the depression (I believe job was a big contributor to my mental state. But I really want to part on good terms. Any advice re wording?

Honestly If I were you and wanted to keep my employability as high as possible, i would be writing something like
"Dear xxxxxx,
I am writing to you to formally tender my resignation. My final working day will be (whatever you think it should be)
(if you have anything positive to say add it here)
Yours Sincerely (or faithfully if you have started the letter dear sir or dear madam)

If you cite depression as a part of the reason for your resignation, while any firm who you apply to cannot use an illness issue as a reason not to employ you, it can be hard to prove and might go against you. Of course you will answer honestly any question put to you by a prospective employer but no point doing anything that might lessen your chances is there?

Well my employer knows about the depression anyway but yeah I guess you're both right. As to what I want to achieve ... I just want them to realise that it's not "my fault" I'm leaving rather it is the illness and also that I believe a big "part" (not all admittedly) of the illness was

Well I wouldn't tell them i'm leaving with depression and I'm also starting a new job.
I'm not sure they'll be happy to keep paying you sick pay during your notice period if they know you are working for someone else, but in practice I think they'll just pay what's due and accept that things didn't work out between you
