my g/f had carpal tunnel surgery about a year ago and it has come back . they said she may have to ubdergo a more serious (deeper) operation this time

are there any alternative methods of treatment she could try first

i,e acupuncture / physiotherapy ??

she suffers with type 1 diabetes which i think has contributed to her condition

This is a difficult problem and there are many facets to it' solution.
Firstly, was the initial diagnosis of CTS correct and was there an improvement of symptoms after surgery?

Yes, there is an association between CPS and type 1 diabetes.

Are her present symptoms due to recurrent CPS.?

If the answer to the above questions indicate that it is a recurrence of compression of the median nerve, then re-exploration and division worth a variety of grafts is the only option.

i would say the diagnosis was correct

they were hoping it was something differnt this time but its not

she had electric shocks in her hands on sat when they re tested her
OK...then that answers all my queries.
I suppose one could try and injection of steroid into the tunnel area, but it looks like re-exploration is the definitive answer.
