Doctors records

This may depend on the hospital involved but..........
In July I had a bad accident and was treated at a local hospital. This included almost daily treatment, dressings and drugs. At the end of July I had yet another accident, so back to the local hospital for dressings and drugs.
Come last month I was referred for treatment to a main hospital. So, three lots of treatment as an outpatient and also drugs included. With me so far?
In seeing the nurse at my Surgery last week I asked if they had the details from 1st July on my file - they don't! I think something is very wrong as some of my medical records are not on my file - and when drugs are involved there's no record to another doctors prescription or even the fact that I had a few accidents. Would this be right? I really would have thought in the day of computer records that my details would have ended up on my Doctors records for me so she would have known what I had been up to.
