Smoking also leaves carbon monoxide in haemoglobin - carbon monoxide will displace oxygen, reducing the amount of oxygen being carried by the haemoglobin through the venous system that is available for tissue & organ perfusion, including the heart. Because of the lack of oxygen being perfused throughout the body, the heart will try to beat faster in order to pump more blood, the heart itself is not getting enough oxygen, vicious circle, e.t.c All these factors mentioned will put added strain on a persons heart, possibly culminating in a heart attack.
