What's wrong with my car?

I've got a problem with my car in that whenever I change into 3rd gear or above and acclerate too quickly, the rev counter shoots up, because the car keeps coming out of gear. I've also noticed that it's very sluggish if I'm travelling uphill. I think I may need a new clutch but I'm not sure of the lifespan of a clutch. Any suggestions? Thanks.

You need to get this looked at asap for two reasons 1) some clutches are adjustable and may be saveable if caught soon enough 2) if left to long will also damage the flywheel and that will be a added repair costs. Things like clutches and brakes are wear & tear items and notorious for give lifespan range as it depends on so many factors.
