The tube is a reservoir of the basic toothpaste blend. The exit has a small circular bladder (sort of like a common flat washer) of another tinted ingredient surrounding the nozzle, with small ports spaced around its circumference. When you squeeze the tube it exerts pressure on the bladder which causes the tint to ooze out making a line of color as the paste extrudes by.
