Yes and you can take your prescription to any opticians
They'll usually just give you a copy - just ask nicely!

You are legally entitled to a copy of your prescription - most give you it automatically.

Don't know about seeing his actual records though.
legally you are entitled to a copy of your prescription, and to see your notes, just like in a doctors, when you ask.
