What happens when someone who was an LGV license reaches 65? I know they have to take a medical but how frequently do they have to take them after that? is it every year?

Yes - the licence must be renewed every year, and that includes a medical.

And of course, between 45 and 65 you have to renew your licence/take a medical every 5 years.
