This is the sort of clue scorpiojo would normally solve.
I've run out of ideas- I can see lots of 11 letter words that cover parts of the clue or maybe the definition but I can't pull it all together.
Time to give up i think

As you say, a ladybird can be a Pen, Bird is a slang term for prison which in the US is a Pen. Pure interest really.

If 'sounds like', h Enlightened fits part, but as ff says, several eleven letter words also partly fit. Think I'll give in to this one.

Well prehensions contains the HEN part of the clue (lady bird), mallyh, but I can't parse the rest of it. I also wondered about COMPREHENDS but can't explain that either.
Other words that may be the definition are:
PROBABILITY- but can't see any word play
ANTICIPATED -ant being a lady bug?)

PREMONITION -can't explain that except PEN is sort of in there)
FORECASTING - F for female+ sounds like AUK + a sting (gives you a spotted rash), but it seems like the tense requires 'forecasted'

Is PEN ET RATION (rash on) worth another look?

If anyone else had this thread bookmarked, emperor has informed us that the answer was MINDFULNESS so we were generally on the wrong track with jem/pen etc. (The link seems as if it could be to the recent Ladybird Book of Mindfulness book.)
