If it were ill the first thing you would do is contact a vetinary surgeon?  I don't fully understand your question.

If you suspect that a pony is being ill treated or neglected and may be dying, then should should immediately contact the RSPCA.

I'd imagine you know pretty soon! They would generally start to look very lethargic and lost their sparkle in their eye and they're coat would generally look poor.
