What is the procedure to change and how long does it take? Do you have to ask to be taken on?

I have been under the same practice since I was born, they kept me on when I moved but I have decided that I should really change to one that is nearer to where I live, any advise?

Go to your local docs and tell receptionist that, they usually just get all you details faxed over from old surgery. Very quick and easy.
Your previous medical details are not faxed over. You need to fill in a registration form at your new doctors and they will send the details to their local health authority. The health authority then recall your notes from your old doctor. This can take a few weeks, depending on how quickly your old doctor forwards your notes to the health authority, who will then send them on to your new doctor.
Well mine were faxed just saying it from my point, obviously all docs are different. So just ask your receptionist.
