It is a rare event- average no deaths per y - 3
and so we can say it follows a Poisson distribution with av 3
chance of 6 alone - 0.05
but we really want to know the chance of 6 and above 0.08
( 7- 0.022, - 7 or above 0.034)
and so boys and girls the alarm should have sounded on numbers alone as soon as the tally struck 6
( you are allowed to foo, and wonder : " poisson, dat stinks, dat does" - and other fab one liners)

How do you diagnose an air embolus?
you X ray them and you can see the bubble in the vascular system. Did none of these children have post mortem x rays?(*)

delete the pathological diagnosis of air embolus, pre and post mortem and make me laugh!
Ditto for the distribution of deaths over a year (13) and whether they should have pressed the alarm button sooner, when the tally reached 6
